1). How old are you? 37/36 at diagnoses
2). What is your race? white
3). What was your diagnosis? Stage IIIa grade 3-metastatic, because it was found in nodes after bi-lat prophylactic mastectomy no original breast tumor.
4). Are you over weight? If yes, how much? 40lbs I weigh 180
5). Do you or did you dye your hair? What color? Brown/ auburn
6). Have you worn acrylic nails? If yes, how long? yes on and off for the last 20 years.
7). Other Chemical exposure note worthy? no
8). Did you have chemo prior to surgery? No If yes, was it due to tumor size?
9). Do you use Splenda? Did not anymore
10). Do you get much sun? If yes, do you use sun screen? Yes and yes
11). What State do you live in? Texas
12). Have you ever worn underwire bras? Yes
13). Which breast had cancer? left arm pit. same side as twin sisters breast tumor.

1). How old are you? 50 1st dx at 39, 2nd at 49
2). What is your race? Caucasion
3). What was your diagnosis? Invasive Ductal Carcinoma-Stage 1 in 1999, now IDC TN Stage III? can't stage correctly nodes were removed 10 yrs ago
4). Are you over weight? If yes, how much? I was 70 pounds overweight put on 72 pounds 10 years ago during chemo and hormonal drugs, now 35 pounds overweight 5). Do you or did you dye your hair? NO

7). Other Chemical exposure note worthy? lived around apple orchards all my life, pesticides 8). Did you have chemo prior to surgery? No

2). What is your race? Caucasian (relatives mostly from northern Europe)
3). What was your diagnosis? TNBC, Stage 1 (see below)
4). Are you over weight? If yes, how much? Average BMI, but probably need to lose 5 to 10 pounds.
5). Do you or did you dye your hair? What color? Yes. I used to get highlights but when the grey started coming in heavier, I died my hair blond.
6). Have you worn acrylic nails? If yes, how long? No.
7). Other Chemical exposure note worthy? None 8). Did you have chemo prior to surgery? If yes, was it due to tumor size? No.
9). Do you use Splenda? Very seldom. Usually only when I'm out at dinner and have coffee, which is not often. I drink tea without any sugar. 10). Do you get much sun? If yes, do you use sun screen? No, but when I do, I use sunscreen faithfully. My mother has had problems with skin cancer from long term exposure.
11). What State do you live in? Missouri 12). Have you ever worn underwire bras? Yes
13). Which breast had cancer? Right
