Dear Suzie,

As a woman with widespread metastases, lung, lymph nodes, pelvic, etc., since March of this year, I know the feeling of having your heart in your throat and your stomach in your toes.  One thing I have learned from my quick study of metastases is that it is better to be in the bones than organs (not easy to appreciate when you do not want any of it anywhere - but that is what we are told).  The first thing you have to do is to get control of your pain.  You lose your clear-headed focus to get your treatment plan in order if you are in pain.  My kind and compassionate oncologist taught me this valuable lesson and she was absolutely correct.  You will be pleased to find that the bisphosphonates have a remarkable effect on the bone pain.  I am currently on clodronate (Clasteon) it is an oral bisphosphonate. They have injectables also that you may have.  The bisphosphonate circles the 'break' and strengthens the bone so it does not fracture again. The bisphosphonate is also thought to have anti-tumor properties also.   The bisphosphonate is also a great pain reliever, along with an anti-inflammatory you should be hopefully be comfortable.  Please ensure that your oncologist finds you an orthopedic surgeon that deals with pathological fractures (those caused by tumors) to have the past healing outcomes and alignment of your bones.  There are new treatment options in the field of radiology to deal with bone metastases and hopefully your radiation oncologist is aware of these.  If not, we will provide you what you need to share with your treating physicians.

It is a great shock I know, but we will help you through this next hurdle.

Love,

Connie

Suzie,

I am so very, very sorry to hear about your arm and am totally shocked by your post.  I thank God that Connie is back with us and has been able to give you the wonderful advice she posted above.  I feel as though we are all suddenly being blind-sided by this freaking disease - I just want to scream "What the HEll is going on here"????  When one of us gets news we weren't expecting or ever hoping to hear again, it effects each and every one of us on this board - to the core  - as no one on the face of this earth, no matter how much we love them or they love us, can feel  or better understand the fear, depression and sadness that comes along with this news, than another that is walking on the same path, sharing the same hope that maybe this has been beaten down forever, and then to find it's reared its ugly head again.

Please know that you are in my thoughts and in my heart.  I wish for you all the best and that a treatment plan is started as soon as possible.

Suzie,

I am shocked and saddened to hear your news.  I know you've been in pain for some time and was hoping it was not what you feared.   We are here for you and will help you out in anyway we can.  I've said this before, but I don't know what I would have done without my friends on this forum to help and hold me up every step of the way in fighting this horrible disease.

Sending you a great big hug,

Suzie,
I'm so sorry to hear about your arm.  sh*t, double sh*t, triple sh*t.    I hate cancer.  It is NOT supposed to start in a far extremity like a radius.   Well, Connie's advice is excellent.  If you have to have a met, bone is the place for it to be.  If you go to the website online community Inspire For Advanced Breast Cancer (google it), you will find that there are many many women who've been living with bone mets for years, and I mean LOTS of years.  15, 20, and so on.   So don't count yourself out yet.  This may well be a chronic condition to be dealt with (true with visceral mets too but even more so with bone mets)   As Connie says, the first thing they are likely to do is put you on a bisphosophate.  Zometa is the drug of choice for this in the US.   Beyond that, you'll have to see what the recommendation is.   And as Connie says don't be shy about treating your pain.

We're here for you, as Donna says every step of the way.

Much love,
Denise

Linda you write so much of what I am thinking but can't put into words as you do.  It does make one feel helpless when we continue to read of others, our friends and sisters in this forum who keep learning of their cancer coming back.  Typing something to show our support makes me feel so lame sometimes, I wish I could do so much more to show how much I care, to make things better, help our sisters to feel better..it is important to remember we have many women in this forum who have had their disease progress that are doing very well and living many years.

Suzie,

Please talk to your onc about bisphophonates to help with the bone pain.  I found this research article on Denosumab (Prolia).  I'd also ask your onc about Denosumab.

Denosumab Delays Bone Complications in Women with Metastatic Breast Cancer

Among women with bone metastases from breast cancer, denosumab may be more effective than Zometa® (zoledronic acid) at delaying or preventing bone complications such as fracture. The results of this Phase III clinical trial were published in the Journal of Clinical Oncology.

Metastatic cancer refers to cancer that has spread to distant sites in the body. Several types of cancer—including breast cancer—have a tendency to spread to the bone. Bone metastases can lead to serious problems such as fracture and spinal cord compression and may require treatment with surgery or radiation therapy.

Bisphosphonate drugs such as Zometa are commonly used to reduce the risk of complications from bone metastases. Researchers continue, however, to explore new approaches to treatment.

Denosumab is a drug that targets a protein known as the RANK ligand. This protein regulates the activity of osteoclasts (cells that break down bone). Denosumab has been approved for the treatment of postmenopausal osteoporosis and has also shown promising results in the management of bone metastases and treatment-related bone loss.

To directly compare denosumab to Zometa among breast cancer patients with bone metastases, researchers conducted a Phase III clinical trial among more than 2,000 patients. Study participants were assigned to receive either denosumab or Zometa.

The objective of the study was to determine whether the occurrence of bone complications (“skeletal related events”) differed between the two study groups. The bone complications that were evaluated were fracture, radiation to the bone, surgery to the bone, and spinal cord compression.

  • Patients treated with denosumab remained free of bone complications longer than patients treated with Zometa.
  • Overall survival and time to cancer progression were similar among patients treated with Zometa and patients treated with denosumab.
  • Osteonecrosis of the jaw (an uncommon but serious side effect) was seen infrequently in both study groups (2% of patients treated with denosumab and 1.4% of patients treated with Zometa).

The results of this study suggest that denosumab may be more effective than Zometa at delaying or preventing skeletal complications in breast cancer patients with bone metastases.

Team Suzie, I like that!

I have been thinking about you, this may be off the wall as I am not a white coat but could there be a possibility the bone cancer could be a new cancer as opposed to bc spreading there?  I know you will learn everything soon thank heavens, so glad you don't have a long wait, the week-end is bad enough.

Suzie, Pam brings about a good point to ask if this could be a new cancer in the bone. You could also inquire if you should have a p53 gene mutation test - a positive mutation is often but not always associated with Li Fraumeni Syndrome - a disorder that greatly increases risk of developing several different cancers in one's life time - people who have the p53 mutation are discouraged for having raditation if at all possible as it can create very  unstable cell states and could trigger new cancers.  The test is usually given if you and/or several family members developed certain cancers at a fairly young age.

Sorry you have so much to deal with. One of my BC   tumours was also metaplastic and TN - I wish there was more they could give us for this in terms of screening and treatment.

Yes, good idea, this way we can all keep track of how everyone is doing - add specific things we learn that may help them, etc.  Every one of our gals experiencing a problem should start their own thread, it would be helpful to all and this way something important for one won't get buried in someone else's thread.

I like your idea, Pam.  It makes great sense.

Suzie,

I changed the topic name of one I created before.  OK, I posted something and then tried it out and it didn't work.  You will need to go to the very first post on this thread you created.  Then click on Post Options, then Edit Post.  It should bring up your original post where you typed the subject heading.  Just change it to your new thread heading.  I hope this works, let me know if it doesn't.

HugYesterday was a defining moment...met w/Onc w/inadequate path report....adenocarcinoma was IT...only could say this was definitely from the TNBC w/mets to bone and alot in lungs...inadequate amount of biopsy tissue to evaluate what WE look for to roll up our sleeves and dive into making strategic tx decisions to keep this beast at bay and be the healthiest I can be from here on....

She could only say it was from the bc, not another cancer.  Since this is one of THE centers in the world, I blew up and had a melt down....pulled the team together, asked she send it to another lab for the hopes of at least more specifics to recommend other than Doxil and Cisto/Carboplatin indefinitely....and I already have heart issues and hadn't done Adriamycin.  So, I am seeing the cardio onc next week while I go through radiation for my broken arm this/next/next week.

Also starting the Metastatic support group, see the Onc....she looked so defeated and had so little to offer after coming off the conference in TX....now biophosphonates...not very promising either.

What have others done for chemo to do say, by pill to stave off liver involvment and spread when I am the worst side effects person on chemo!  I did it on cytoxan/taxotere thinking every horrible side effect was for my greater good....not so.

I asked if scans would have changed this progression....no, still would've progressed to stage iv.  I feel pretty good considering the broken forearm on pain meds and surprised to feel so well when the lung path looks pretty awful....

I put here I have pushed for my SSI app to be fast tracked so I can stop worrying about money.  I met w/their psychiatrist, gave me another dose of anti anxiety meds/day.

She says average time...2 years....but, I simply state....I have NOW and want to stay feeling as well as possible to be with/do with family/friends whatever the time.

I imply NO pessimissm here for me or you all....but, I hope what I get for Christmas is another Christmas, you know?  Picking out new clothes, scarves and a girly spa time for me and my daughters.....my heart is breaking, but I am strong....carry me and I'll carry you...big group hug

Suzie,
I had my annual bone scan last week and they saw a spot on two of my ribs. I had taken a fall a few weeks back and assuming that's when they were fractured. Only thing that worries me is that I felt no pain.
Anyway, my onc saw the report and ordered xrays to the particular area and I will get the results of those Monday when I see him for my scheduled appt.
Soooo here we go again....

I don't get around to all the boards the way I should.
Helen, Now I understand your question about xrays....that was back in the middle of Dec. Everything is fine. Onc going to look at the ribs again by CT in March. But he is almost certain it's due to the fall.

Dear Suzie,

I had the pleasure of meeting Pam about 18 months ago about an hour outside of Dallas. Had dinner with her, her daughter and DH.

I can tell you that she is truly a marvelous women...In spite of several difficult medical issues she gives her heart and soul to this site and has helped so many, many of us with her diligent research on our behalf and with her insightful posts.

She is very smart, caring and funny...Not a bad combo, huh? I would say her only failing is that she likes to sit under palm trees on the beach with their fronds gently blowing in the breeze...not an easy task in January in Oklahoma...Smile

She is a lovely woman..I can see her now..shaking her head and saying “why is he saying so much?” well the answer, dear Pam, as Suzie and all of us have recognized you are just so very, very special and I want to acknowledge that. I hope 2011 is kind to you, health-wise..

with my love,

Steve

Beth and D...

My arm is responding to rads...just finished tx.  Some pain and swelling, but I think the broken arm rads helped and doing another scan in 4 wks is the plan to see the healing.  Still in sling, elevated, very carefully not moved.  The OTHER arm is now daily acting the sameConfused...call into onc while nurse cares for the pain...I am now the queen of pillows...lol, piled left and right!  Rad Onc has passed the baton to Medical BC Onc now...since we have to watch for more bone mets/sites and want to discuss the zometa alternative as part of tx, and the lung mets, seeing the Onc makes sense...now systemic tx/chemo...whole other question/getting second opinion on case/tx with the help of my dear friends here.....I thank you everyday....how are both of you??????

Suzie, I'm glad the radiation seems to be helping, and hope the healing has already started.  Sorry about the other arm, though.  Hopefully it's just strained from using it to compensate for your 'queenly' lady of leisure arm.

I'm doing pretty well, other than walking around with a cold that turned into a sinus infection and now back into a cold/flu since antibiotics.   I'm slurping soup at my desk at this very moment.  We had a nice holiday break, going to LA for a week and leaving the kids with the grandparents for a while when we returned.  Lots of books, movies, hanging with friends, running, and yoga, the latter two of which have been nixed since the cold-monster came to visit.   My marriage seems to have taken a turn for the better, amazingly enough.  Our 15 year wedding anniversary is tomorrow, and we are both feeling pretty optimistic.    My chest is still less than lovely with my pretty botched reconstruction, but I don't want to revisit this quite yet.  Maybe in a couple of months.    Thanks for asking.

love,
d

Man, I have a terrible memory but I remember someone on the spiritual support mentioning a botched reconstruction, but I don't know who....of course my problem is chemobrain, I would never admit to being old...lol...
Happy to hear there are positive signs at your house...I will pray that all is well and soon.
Love,
Carol

Denise,
Hope that cold finally goes away.  Mine is hanging on way too long.  It's nice to hear about your trip to LA without kids.  Hoping that things continue to look up in the marriage area!  Congratulations a day early on your 15 yr anniversary.

Yes, that chemobrain is all too real.  And as my family/friends (here as well) say, meds in the mix are just "SO" much fun...not!  Tennessee is so beautiful, springs are lovely there, having lived in SW Ohio years back.  Stick w/us and we WILL always remember what is worth saving and with a breath of kindness, blow the rest away   hugsHug

My user name I picked when I was doing foster care.  It was a zoo around here and I'm a mommy, so I became zoomommy2 because somebody else already had plain zoomommy.  I do foster care/rescue for dogs now, so the zoo continues!
