I just talked to my oncologist.  It appears that the spot that lit up in the PET scan and showed on the MRI was an internal mammary lymph node.  One node is enlarged, about 1".  The problem is it's in a tricky area behind the rib cage and hard to get at.  She's going to be talking to some surgeons/thoracic surgeons on how best to biopsy the node.  I know that it may be something other than a recurrence until we have the biopsy and know for sure.  However, my onc's going at this as if it's a recurrence, which would mean radiation and possibly chemo againCry.

Have any of you had a recurrence in this area?  What kind of treatment did you have?  Did it clear it (NED)?

What do I need to know or ask my medical team?  Any help would be appreciated.

I am sorry.  I did have a recurrance there and did chemo and radiation.  It did clear up though.

Hi Donna, I did have recurrence in mammory node. It was grown to about 4-5 cm before it was removed surgically. I had toracic surgeon to remove it. The tumor was between 2nd and 3rd ribs and also next to sternal bone. The operation was not small, but my surgeon did excellent job. He had to cut off parts of the two ribs and sternal bone. After the operation, I had radiation and chemo. The tumor area is cleared after those treatment. However, the recent PET/CT discovered that 2 nodules in my lungs. I am current under chemo and was just selected in PARP trial last week.  I hope and pray for you that your biopsy will find the enlarged node is not cancerous.

I hope everythingh is benign but my mind immediately goes to the worst case scenario whenever they do/find anything. I had radiation after chemo and I asked if they had radiated he internal mammary nodes because in the studies I had found high grade tumors with lymph node involvement (my Ki67 was 80%) needed to be careful about those nodes. He seemed surprised when I asked and like so many of my docs didn't give me a straight answer. I can't have radiation again,too close to my heart and I've already  done it on the left side. I'd ask cam on edges about chemo therapy types. I have found him to be direct and explicit with his recommendations and explanations. Something I appreciate.
I don't post much but I follow yours. We were dx at the same time(I was 8/09) but you've become such a force here on the forum. You've helped me several times with the studies you post and you're always so gently positive with anyone who posts.
However it goes I know you have a strong cheering section here. My thoughts ad prayers as well.

Christine,

How long ago did you have chemo and radiation on those nodes?  What type of chemo did you receive?  What type of follow up care are your receiving?  Was it difficult to biopsy the node?

Harbin,

Do you think the recent lung nodules are the result of the mammory node?  How long ago did you have the surgery for the mammory node?  Do they monitor that area closely?  What type of chemo did you get after your surgery?

Hi Donna - I am so sorry the PET/CT scan came back with the mammory node lit.  We spoke last week and as  I said then my prayers are with you that once biopsied it will come back okay.  Please try to keep positive, easier said than done I know, but I am sending prayers and positive thoughts your way.  When you went for the initial MRI before the PET/CT was that a routine MRI done annually or was it because of the pain you had been feeling on the one side you were experiencing?   Please keep us posted as you find out more about when they will biopsy it, etc.  Please know that we are all here for you, as you have been for everyone on this site.

Thanks all for the well wishes and prayers.  I'm having the biopsy on the 15th and should know the results by the 17th or 18th.  I guess I won't know for sure what it is until it's biopsied.  This latest Pet scan was a routine scan, nothing symptomatic.  The MRI was because the area on the PET that lit up needed to be looked at closer so I had a contrast MRI.  Prior to my bi-mx surgery initially, I had an MRI and  CT scan of the chest.   Both of those tests were clear for anything else besides the original tumor.  I feel better that my onc told me she has a plan either way.  I feel we're on top of things for now and just waiting for the biopsy and to find out the results.

Oh Donna,
Oh ugh.  Ugh, ugh, ugh.  I'm so sorry about this, but HOPEFULLY it will be benign!!!  I originally had an IM node light up on my initial PET scan, but it was thought (hoped!) to be just inflammation from surgery, a recent sinus infection, or an anomaly.  They thought mine was in an area that was inaccessible so it was never biopsied, tested, or removed.  I didn't have another PET scan until after chemo/rads, and it had disappeared, so that's what I'm wishing for you too - that it just disappears!  Any chance you've had any kind of recent infection or anything?  Please do keep us updated and know we're thinking about you!

That's it - I'm giving you back the crown!  You be False Alarm Queen - it sure beats the alternative! Wear it well!

Thanks, I'll take the crown anytime!  I did have a bad sinus infection/bronchitis in September that took a long time to get over.  I was on the Zpack, bronchial inhaler and mucinex.  I've been much better for about 3 weeks and no longer having symptoms.  It's comforting to know you has suspicious nodes and now they're clear.  Great news!

Donna, I've got my fingers crossed for you. The waiting is very hard, I know. I hope you're able to find some comfort and distraction these next few weeks.

(I do have mets to my internal mammary nodes but only the lung met was biopsied, so I don't have any concrete info for you about how that works.)

Ohmigosh, Donna - I'm SOOO glad you had a recent severe sinus infection, LOL!!  I'm sure you know what I mean - just that I want THAT to be the reason for your IM node to be suspicious, like mine was!  I don't remember - did you have rads before?  They included my IM node in my rads since they weren't able to biopsy it.  With the IM node plus my left-upper-inner tumor, I was concerned about heart/lung damage from the rads - but as far as I know there are no significant effects so far.  My MUGA scan to check my heart after treatment was very good, and I think I have some very minimal lung damage, but in the scheme of things I don't mind.

In any case, I'm sending good vibes your way!  Hang in there and try to distract yourself as much as possible while you wait!   Got some good chick flicks to watch for distraction?

ETA-My double MX was delayed for several weeks because of....guess what.... a severe sinus infection, so I don't mean to downplay the misery of that!

Hi Donna, I think the mammory node and lung nodules are bot the met from TNBC. Although no one knows for sure how they happened. The mets could pop up anytime and anyplace when they want.
The tumor in the node was shown in January and I had the operation in April. (It was mis-diagnosed initially, it's a long story). My doc. has been monitoring me incl. the surgical area closely. I get PET/CT scan every 3 weeks. I had cisplatin for chemo after the surgery. Now I am on PARP. Please let me know if you have more questions. It looks like you have a great doc. but I really hope your finding is nothing serious. ALl the best.

Donna,

I am hoping for the best for you - I had hot nodes on the right side (cancer was on the left). These were discovered when I had a PET just prior to chemo. It was decided that they were reactive due to the flu shot that I had received just a few weeks before. Now, however, I'm extremely nervous. My Ki67 was 97%. Although I have had a few MRI's here and there I have not had a pet scan in quite a long time. I'm not due back to Borson's office until Feb. It's the quiet, lurking stuff that scares the begeebees out of me. I'm definitely going to talk to her about the possibility of a PET scan.

Truly hoping that your situation is b9. My thoughts and prayers will be with you. Hang in there!!

I know how you feel.  I'd definitely talk to Dr. Borson about your concerns and maybe getting a PET scan if it will help relieve some fears.  You're three years out!  Congratulations.

You made me think of something else, I had a flu shot last month.  I know I'm grasping at straws, but trying to think of everything that may be the cause except the worst one.

Donna:  Sorry to hear that you are dealing with this.  Ashley did have internal mammary lymph nodes light up at initial diagnosis.  AC and Ixempra didn't seem to help, but the Carpoplatin and Gemzar seemed to for about 3 months, but at 6 months they were glowing brighter than before.  We have more scans scheduled for November 22.  Our onc and surgeon really never gave us surgery as an option because they said it would be thoracic surgery and would delay Ashley getting started on chemo.  Better to let the chemo and radiation take care of those nodes.  Well, that was back in September 09, and the chemo did not take care of it, and she has never made it to radiation.  I think I'll ask about surgery again at our next appointment.  I'm praying for your biopsy results to be benign!

Donna, I remember when you said you had the flu shot!!!!! I forgot about the sinus infection????
Maybe these are residuals from the infections in your body during this time.?????

Tomorrow's the biopsy day.  I won't know the results until I meet with my onc on Thursday.  Just a few more days of agonizing!  Ativan is helping me get through this!

3 John 2  Beloved, I pray that you may prosper in all things and be in health, just as your soul prospers.
 
It does seem likely to me a node could get inflamed from a sinus infection or a flu shot.  But, I'm not a Dr., but when we have colds, a lymph gland can become enlarged in the neck.  Anyway, praying for a CANCER-FREE biopsy!

I love you all!  I could feel your prayers and B9 chants today.  It helped support and lift me up during the procedure.

The procedure went smoothly.  I was worried how this doc could get into the area under my implant.  Once we got to the hospital, the nurse said the interventional radiologist was wonderful and I was lucky to have him today.  I explained he was the only one my onc wanted to do the procedure.  She replied, I know why.  It made me feel that I was in good hands with a skilled surgeon.

So I'll explain the procedure in laymen's term.  Like I said, I was very curious how they could do this biopsy.  They took me into the surgery room and put me on the CT scan table.  They gave me fentanyl that helped relax and make me sleepy.  I was awake during the procedure.  I think they gave me a shot in the center of the chest to numb it, but I can't be sure because I never looked.  They had me laying more on my side with my right arm above my head.   While doing the procedure he had the CT scan to show him exactly where the needle was going.  He used a thin needle and inserted it in the center of my chest and went sideways under the implant into the mammary node.  He did this 5 or 6 times poking around and aspirating the node.  Once the procedure was done, they took me back to the room where I ate lunch and waited about 2 hours before going home.  I was still feeling the effects of the sedative and slept much of the afternoon away.  I feel more alert now, but still a little tired.  The center area of my chest hurts a little, like someone was poking around with a needle, lol!  They told me to take it easy today and the next 2 days.  I can return to work tomorrow.

I love your B9 chants and prayers.  I'm praying for the same results.   I have to wait until late in the day on Thursday to get the results.  My onc is squeezing me in as the last appointment of the day.  Until then, I'm thinking B9, B9, B9!

Thanks everyone.  It's been a rough road.  I had the PET scan Oct 20th and now it's almost a month later. . .  I'm trying to remain positive, but have to tell you I'm expecting the worst.  I know my onc said she wouldn't tell me anything over the phone and wanted to see me.  However, when I didn't hear anything today or yesterday my mind is telling me it's only bad news.  Why else wouldn't they call and say it's not C?  My appt isn't until 4 pm and she squeezed this appt in so I bet I won't get in until closer to 5pm.  Tomorrow will be rough at work but it will keep me busy.  My hubby is going to pick me up and drive me to the appointment.  

On top of all of this I'm still having mom issues.  Since my dx, I've been dealing with a mom who has dementia/early onset-mild Alzheimer's.  It went really bad last December and I had to file for guardianship.  Well things were getting better and last month we moved her into an independent apt living facility associated with a nursing home where she'd get her meals and be safe.  She seemed to turn around and was almost herself again.  Today it all went south again with her accusing me of paying off the doctors, etc.  I think I'm going to let my hubby deal with this mess.  I can't handle it right now.

Donna:
I am so sorry your mom is sliding back with her dementia Unhappy I am sure you are out of your mind with concern for her and yourself...Life just keeps on giving no matter if we are ready or not...but just focus on your appointment tomorrow and go from there.  Remember, your mom is not responsible for what she is saying right now and you do know the difference.  She would be mortified to know what she has done to hurt you if she were in her right mind.  She does love you, but she is probably scared too not knowing what is happening to her.  I know it doesn't sound like something to let go of but this disease is awful too, almost as bad as our TN status...but at least she will be ok, it is the loved ones who get hurt the most from that disease. 
 
My two good friends went through this with their parents and it was dreadful, but they did get through it and in the end, they remember the funny stuff because the hateful things that were said, didn't really matter anymore and you will come to that point too.  It is just bad timing...
Yes, get your DH to deal with this for the moment.   
 
I will be thinking about you at 5pm your time and I know the angels will be surrounding you and keeping you close to their hearts.

I wanted to tell you neither my surgeon or oncologist give news on the phone (good or bad). It seems to be the way they operate and I have had to respect that. I have received some really good news in the past only after anguishing for days....... The good thing is I get to ask a lot of questions that may be on my mind.

I worry about my parents a lot also.  That is the hard part about getting our cancer as younger women.  I actually feel worse about my parents most of the time because of my health issues than I do about having the cancer.

I went through the Alzheimer's routine with my MIL.  It isn't pretty, but thankfully, your husband can help out.  Try not to take anything she says or does personally because she probably won't remember it the next time you see her.  Praying today for you and your appointment.  I think it's likely the other way around.  If it was bad news the doc wouldn't wait so long to tell you.

Donna, whatever this turns out to be, just remember that the community will help relieve the stress of the understanding...we'll research, ask our own doctors and look around the internet to help you get the best information possible. Please feel free to give your doc's office a little talking to about keeping you waiting so long.
 
As for the mom journey, you know my feelings and I am also a sincere believer that stress is a more powerful fuel to open the door for recurrence than anything else. If you husband can step in to help out (what a nice guy!) or one of the attorneys to give you a break, it will go a long way. You know what you have to do....and if you get some spare time to watch the movie "Hanging up" it is hillariously funny and so profound for those of us battling with parents' with Alzheimer's.

Well I wish I could give you good news, but it is what it is.  I have a local/regional recurrence in the internal mammary node.  Yes, it's very rare to get an isolated recurrence in the node.  My onc believes it was from the original tumor and that there were probably a few cells in the node and was never picked up on the original MRI.  The chemo wasn't effective wiping it out or allowed a few cells to remain to grow again.  The encouraging part is that it's about 14 months post chemo or 17 months post surgery and the cancer is still very small.  She said it wouldn't have been caught on a CT scan and we were fortunate for it to show on the PET.

So here's the recommended plan of action.  She wants to throw the big guns at it and enroll me in the clinical trial of Iniparib in Combination with Gemcitabine/Carboplatin.  After chemo, of which we'll do periodic scans to see if the node clears up, I'll have radiation to the node.  The parp trial is the lottery so there's no guarantee I'll get in right away.  However, I think we can still do the chemo and add the parp when I get chosen from the lottery.

I'm getting a second opinion also.  I'll be calling tomorrow to arrange for it.  Depending on what I decide, I most likely will start chemo again in a couple weeks.  This time I'll need a port and most likely will lose my hair again.  Yuck.

So my dear brilliant friends, any suggestions, ideas, questions to ask? 

I know you'll all be here for me and help pick me up when I need it most.  Right now I know I'm not thinking all that clearly.  My dh and sons took the news worse than me.  My hubby has been by my side through all this and I'm more concerned how it's impacting him.  I think I was prepared for it and this past month of waiting was the most tortuous time, similar to the first dx.  I'm actually calmer now I have an answer.  I knew the moment my onc walked into the office and looked me in the eye what the diagnosis was.   She spent a good hour with us answering all our questions.  She truly is a special woman and dedicated doctor.

I'm so sorry to hear your news.  I was so hoping that you would not have to deal with this again.  I wish I had words of wisdom or advice to give but I will be here to listen and offer support as best I can.

I'm sorry, Donna.  I was praying for better news.

The only advice I have is on the port - make sure it's anchored well.  Mine likes to roll and it's a PIA to gain access to it.  I have one nurse here that can hit it every time.  Everyone else has to poke my chest 3 to 5 times to get in and sometimes then, they don't and I make them quit.

I too am sorry you have to deal with the chemo and radiation again. I know what you mean about being calmer now that you have an answer. It's hard to say good-bad, lucky-unlucky when talking about cancer, but it does sound as though it could have been a lot worse.

I am stunned! Numb! Speechless! But I am here for ya!
 
I hate that you have to go through all this again, but it is doable and you are a wonderful, strong, vibrant, loving, mom, wife, daughter and friend that has a lot of support and love returned to you!!!  Keep the calm you have and put one foot in front of the other and before you know it, you will have beat this beast.......
 
I send my love and deepest regrets to your loving husband and children that our prayers and strong wishes/vibes could not stop this monster......but their love and support will be what makes this terrible cancer journey tolerable.  I respect all that they have done for you and continue to do, as you know, there are lots out there who don't stand by their wives and mom's when there is any tell tale sign of illness...So Kudos to your family!

This truly stinks!!!!!!!  I was just certain all would be ok or you wouldn't have had to wait so long!  It's your body and you somehow just knew! I think our men folk do take things harder than we do.  Praying for them and you, of course!  You know you are going to beat this again!  Who needs hair, anyway!  I'm still fussing with how to fix mine after so many months without it.  I'm glad you feel confident in your doctor, but a second opinion is always a good thing.  The whole forum is here for you, Donna.

$hit.  I truly was not expecting this and I am so sorry.  Your onc sounds great and I am glad you have her and that a plan has already been established.  It is now time for all of us to shift into a gear we hoped we wouldn't have to, and go on from here.  You will get through this - remember - just like the first time - this is the worst of times.  You will get this sucker - and kill whats in there.  Please, remember to make sure they give you a Power Port - that can be used for many scans, etc.  I didn't have to ask for mine, thank goodness, because I was so naive back then, I wouldn't have known to ask, but just in case, make sure you get that.  I hope and pray the next few days are easy on you and your husband and sons - this is so very, very hard on all of you. 
 
I wish this never happened, it is all of our worst fear - to hear it's back - or never left completely.  I can't remember if you had a breast MRI at the beginning of the original diagnose or not.  And if you had, would this have shown up on that?  Oh, God, I guess we can question every thing about this disease to death, yet what the hay does it matter in the end.  There really just are no concrete answers to anything about it's course.  Stay strong, Donna, we love you.

My apologies for my rambling post - I am just truly blown away and I'd be lying if I didn't say it scares the hell out of me too.  Just goes to show - none of us are secure - no matter what we do.  I hate this freaking disease.

Yes I had a contrast breast MRI prior to surgery and all the nodes were clear.  This time the MRI showed the mammary node after I had the PET scan.  I know we all begin to second guess.  Maybe I should have had 6 treatments instead of 4.  Who knows.  Maybe none of this would have mattered anyway.  All it takes is a few rogue cells to hang out and decide to multiply.

Donna, I just sent you a pm.  You are going to kick this b*tch to the curb.  
We love you, and definitely have your back.   Your treatment plan sounds fantastic, and I think you'll find quite livable too.  No or little hairloss, decent energy, and maximum effectiveness.  The women I know who've been on the protocol (assuming weekly carbo/gemzar) have worked and done all their usual activities with few side effects other than low wbc and platelets after a while.

Wow Denise,  thank you for your comforting words about the treatment plan.  We feel we have one more chance at this cr*p and want to take the best treatment possible.  My onc wants us to consider the big guns and hubby is all for it.  As far as having the node removed surgically, it may not be possible or without complicated surgery.  I believe the internal mammary node is right under my implant behind the rib cage.  It would involve a thoracic surgeon and I don't know if there is a benefit to removing it vs. radiation.  She never talked about surgery, just chemo and radiation.

What's depressing to me was the first time around I was told I had a 13% chance of recurrence after chemo.  I was thinking my odds were pretty good, huh?  Right.  Well now we're back in the trenches going after this b*stard TNBC and I'm scared that the treatment might not work.  I don't believe in percentages anymore.
